*Benefit Eligible Employees working 20+ hours per week ages 18-70 can apply with Guarantee Issue up to $100,000 of Life Insurance Benefit. Employees ages 18-75 can apply up to $125,000 with medical underwriting. Spouses/Registered Domestic Partners require Underwriting consideration for all levels of coverage. Participation requirement of 10 approved employee applications and 70% active enrollment needed to offer Guaranteed Issue.
